Hawaii Health Department Cites Five Star Transportation for HI-5 Violations
HONOLULU -- The Hawai‘i State Department of Health (DOH), Office of Solid Waste Management has filed a Complaint and Order against Five Star Transportation for violations of the state’s deposit beverage container (DBC) program. Five Star Transportation, a distributor of deposit beverages that are sold in the state of Hawai‘i, is located at 689 Kakoi Street in Honolulu.
Hawai‘i Revised Statutes (HRS) Section 342G-105 requires deposit beverage distributors to submit monthly distributor reports and payments to the DOH no later than the fifteenth (15th) calendar day of the month following the end of the payment period of the previous month. Five Star Transportation was cited for late payments and reports submitted for the months of August 2006; May, April and October 2007; and May, July, and August 2008.
DOH has assessed a penalty of $3,520 against Five Star Transportation for its failure to comply with DBC program requirements.
